Is Alkaline Phosphatase A Bio-Marker To Predict PSA Decline And Clinical Benefit From Abiraterone Acetate in Bone Metastatic Castration Resistant Prostate Cancer

Again and again I have mentioned the huge need we have for the development of reliable bio-markers. Reliable bio-markers will allow us to know which men will respond positively to a treatment (also those who will not) and when a treatment is still effective (also when it no longer is effective). These biomarkers really don’t exist, so a lot of clinical decision-making is based upon the experience and gut feeling of our doctors. This type of decision-making is not adequate in today’s world of multiple treatment options, especially when many of the treatments can foster resistance to other available treatments. [...]

Belfast Clinical Trial Combining Two Radio-Therapies For Men With Advanced Prostate Cancer

Researchers at Queen’s University in Belfast along with the Belfast Trust are conducting a clinical trial evaluating the combination of two radiotherapy treatments, Radium 223 and Pelvic Radiation using Volumetric Modulated Arc Therapy (VMAT) in men with bone positive advanced prostate cancer. The trial, being called ADRRAD trial, involves combining androgen deprivation therapy (ADT) along with the two radio-therapies. Treatment is expected to last about 18 months. VMAT is an externally delivered type of radiotherapy that manipulates radiation beams to adapt to the shape of the tumor, delivering a more precise dose of radiation and limiting damage to surrounding tissue. Radium 223 is [...]

Levels of Serum Alkaline Phosphatase Reflect Negative Prostate Cancer Bone Activity

Not often discussed, but a valuable measure of prostate cancer bone activity is serum (in the blood) alkaline phosphatase. It is a normal enzyme produced naturally by the body. Everyone produces some of this enzyme regardless of his or her state of health, gender, or age. The tissues that produce the most alkaline phosphatase are the bones and liver.  When prostate cancer spreads to the bones, its most common target, the cancer breaks down the bones and leaks alkaline phosphatase into the blood where it can be measured by a simple and inexpensive blood test. Measuring the levels of alkaline phosphatase in [...]

Transdermal Estrogen Is Good For Hot Flash Mitigation and For Hormone Therapy (ADT)

Yesterday's post about the use of transdermal estrogen patches as an alternative to LHRHa drugs  elicited a question on the Malecare Health Unlocked Advanced Prostate Cancer Online Support Group.  The question raised, or I should say the comment made was that estrogen is used for the purposes of hot flash mitigation and the use of estrogen as an ADT drug was called into question.  This is true, estrogen is sometimes used to mitigate hot flashes, but it is also sometimes used as a first line ADT treatment itself. Originally ADT was accomplished by surgery or the oral use of estrogens [...]
