Side Effects and Toxicity in a Multi-modality Approach Post Radical Prostatectomy with Radiation and ADT

The optimal use and timing of secondary therapy after radical prostatectomy remains controversial despite some new findings supporting the advantages of using aggressive combined secondary therapies was presented at the most recent ASCO GU conference in San Francisco. There are limited data on the patient-reported outcomes following multimodality therapy. Given the move towards increasing combination radiation and hormone therapy post surgery we need to have a better understanding of the possible side effects of this type of treatment modality. Long-term urinary continence and the quality of life as effected by the combined therapies are two of the side effects recently [...]

The Benefit of Using Local Prostate Cancer Therapy for Men with Lymph Node Positive Disease

In an issue of ONCOLOGY, there was a review of the literature provides us with a comprehensive review of the limited literature on management of men with prostate cancer who have pelvic lymph node involvement but no known distant metastatic disease at diagnosis. Common folklore has been that these men, with limited pelvic lymph node involvement, are considered “incurable” and managed solely with androgen deprivation therapy. However, there are several reasons to consider local treatment of men with lymph node positive prostate cancer with prostatectomy, radiation therapy, or both prostatectomy and radiation. Based on the literature we need to change [...]

Adjuvant Radiotherapy Benefits Survival in Subsets of Men With Node-Positive Prostate Cancer

In an analysis of outcomes reported in the Journal of Clinical Oncology, men who were diagnosed with lymph node-positive prostate cancer (no bone involvement) who went on to receive targeted radiation (adjuvant radiotherapy)  enjoyed a survival benefit over those individuals who did not have this therapy. This finding was specifically for men with both low-volume and intermediate-volume  lymph node involvement irrespective of other tumor characteristics. The study, which evaluated 1,107 men with pN1 prostate cancer who had been treated with a radical prostatectomy and anatomically extended pelvic lymph node dissection between 1988 and 2010 at the Mayo Clinic in Rochester, [...]

Using choline-C11 PET/CT Scans To Predict Survival In Men With Recurrent Prostate Cancer

Choline-C11 PET/CT scans can identify the actual sites of recurrence of prostate cancer in men who have a rising PSA after receiving primary therapy. The question worth asking is if this type of scan might be able to be used for prognostic purposes. Knowing this might influence our treatment decisions as well as our basic life decisions. A retrospective analysis was performed of data from almost 200 men who had a radical prostatectomy as their primary prostate cancer treatment and who then received a choline PET/CT after a biochemical failure (PSA rising to a level greater than 0.2 mg/ml) while [...]
