Side Effects and Toxicity in a Multi-modality Approach Post Radical Prostatectomy with Radiation and ADT

The optimal use and timing of secondary therapy after radical prostatectomy remains controversial despite some new findings supporting the advantages of using aggressive combined secondary therapies was presented at the most recent ASCO GU conference in San Francisco. There are limited data on the patient-reported outcomes following multimodality therapy. Given the move towards increasing combination radiation and hormone therapy post surgery we need to have a better understanding of the possible side effects of this type of treatment modality. Long-term urinary continence and the quality of life as effected by the combined therapies are two of the side effects recently [...]

Using Low Dose Prednisone in First-Line Docetaxel Treatment Improves The Quality of Life (QoL) For Men With Prostate Cancer

Clinical trials are used to both demonstrate that a drug or treatment has efficacy (extends life or improves the quality of life). They also are used as the vehicle to obtain approval from the governmental body responsible for monitoring and approving their use. How the trial is set up will directly dictate the eventual approved label (the approved dose as well as any additional drugs or treatments to be administered along side the investigational drug, etc.). This doesn’t necessarily mean that the label is actually the best option for use as other untested doses or co-administered drugs could still be [...]

Our New Therapies and the Development of Cross Resistance to these Drugs

Men with metastatic castrate resistant prostate cancer (mCRPC) have become fortunate over the last few years as there we now have a number of new drugs available which can extend our life and can also have some palliative effects. However, as we have more treatments being approved, we find that we are faced with a significant issue, how to sequence these drugs. The major problem causing this sequencing crisis is cross-resistance. The use of each of any of these drugs limits the use of the another drug in men who have failed the original prior therapy. This problem, cross-resistance, is [...]

Combination Chemotherapy Treatments Maybe More Effective Than Each Drug In Sequence

According to a news release from MD Anderson they have recently completed a study that shows that combining two separate chemotherapy agents is more effective than sequencing them one after another. The current standard of care for the cytotoxic chemotherapy agents used to treat prostate cancer is to treat men with advanced metastatic castration-resistant prostate cancer (mCRPC) by the sequential use of single agent taxanes such as docetaxel and cabazitaxel. Usually, docetaxel is used as the “first-line” therapy while cabazitaxel is used as the “second-line” therapy. The use of these therapies as a combination therapy at the same time has [...]
